Making a referral
Most of The Junction’s services are accessed through a referral system. We need to collect a little bit of information about you and your situation so we can make sure you access the appropriate service. The referral forms for each service can be accessed here. Once a referral has been received by the team the information will be discussed at a referral meeting where a member of staff will be allocated to offer support. That member of the team will be in touch to arrange an Assessment with you or your parent/guardian if you are under the age of 16 and if it’s appropriate for that service. However we know that sometimes you may not want your parents/guardians to fully know your situation so we can accept a referral which is signed by an appropriate adult which could mean a teacher or another relative as long as they are over 18. If you are over 18 this doesn’t apply to you.
